Mission Connexion NW Registration

Mission Connexion Northwest is all about the “Great Commission.” Sponsored by Northwest area churches, it is a two-day event challenging the Church to fulfill the Great Commission. Its three-fold purpose is to inform, celebrate, and challenge people of all ages to get involved in missions by sending, praying, giving, and going.

This year, we will go to Sunset Church in Hillsboro, Or on January 20-21. The cost is $60 per person, which covers transportation, lodging, Friday’s dinner, and Saturday’s breakfast. The attendee must provide their own lunch and dinner on Saturday.

I, being the Parent or Legal Guardian of the above-named student do hereby give my approval for the previously mentioned minor to attend the Mission Connexion NW Event on January 20-21 and sponsored by Toledo Baptist Church (hereafter referred to as the sponsors). I, as the parent/guardian of the above-named youth, assume all risks and hazards incidental to the conduct of the activities during the event. I do hereby release and hold harmless the sponsors, the organizers, and supervisors from any and all loss, injury, or other damage incurred to me or the above-named individual arising out of the event. In case of injury to my child, I hereby waive all claims against the organizers, the sponsors, or any other supervisors appointed by them. I also grant permission to an attending physician or hospital to perform whatever care is necessary for the welfare of my child until such time as you are able to reach me personally.
